This course will be held Sunday, November 16, through Thursday, November 20, at the Dubal Training Center. The welcome reception will take place on Saturday, November 15. The registration fee includes the following:
- Saturday evening welcome reception
- Morning and afternoon refreshment breaks during lecture intermissions
- Luncheons – Sunday through Thursday
- Wednesday evening dinner event
Attendees are required to register and badges must be worn to gain access to the course and social functions.

Cancellation Policy
Requests for cancellations must be made in writing to TMS. A $75 processing fee is charged for all cancellations. No refunds will be processed after the registration deadline of October 20, 2014.
Photography Notice
By registering for the conference, all attendees acknowledge that they may be photographed by TMS personnel while at events and that those photos may be used for promotional purposes.
Audio/Video Recording Policy
TMS reserves the right to all audio and video reproductions of presentations at TMS sponsored meetings. Recording of sessions (audio, video, still photography, etc.) intended for personal use, distribution, publication, or copyright without the express written consent of TMS and the individual authors is strictly prohibited.
