Use this form to register to attend the 1999 TMS Annual Meeting February 28-March 4, 1999, in San Diego, California. The deadline for preregistration is February 12, 1999, to qualify for the advance registration rate. Subsequent to February 12, it will be necessary to register personally at the meeting (at a slightly higher cost). Registrations received after February 12, will be processed using on-site fees. Students must send via mail or fax a copy of their school identification card to receive the student rates. Presenters who have citizenship in economically disadvantaged countries, as determined by the World Bank, and who have a demonstrated financial need may qualify for a registration honorarium.

Refund requests must be in writing directly to TMS and postmarked no later than February 26, 1999. A $50 processing fee will be charged for all cancellations.

NOTE: If you wish to register for a short course or an optional sightseeing tour, it is necessary to complete either the short course registration form or the optional sightseeing tours registration form, presented in portable document format (readable and printable using the free software Adobe Acrobat Reader).

It is extremely important that you fill out this registration form completely and accurately. Incomplete registration forms cannot be processed.
